Request for Permission Numbers Form

Please Note:

The Permission Number Request Form page is maintained by the administrative staff of the Mathematics Department. The administrative staff do not determine student eligibility for permission numbers for course enrollment. We comply fully with the policies and requirements established by the University and the department faculty.

Permission numbers will not be issued to students who have holds on their accounts. Permission numbers are issued only under the following circumstances: (1) the course has available seats and the student satisfies all prerequisite requirements; (2) at the request of the faculty member instructing the course; (3) for courses taught by a teaching assistant, at the request of the course coordinator; or (4) for courses listed with Staff as the instructor, with the approval of the Department Chair.

Permission numbers are issued in the order in which requests are received. Please do not submit multiple requests or resubmit a request unless you are providing additional information to substantiate your appeal following a denial. Please be advised that, during the enrollment period, a requested section may reach capacity before your request is reviewed. Accordingly, submission of a Permission Number Request Form does  NOT guarantee that a permission number will be granted.
